Involving friends or families in the care-plan assessment is important as it allows us to understand the requirements of the individual, especially when communication may be difficult which will enable us to have the same goals in mind.

Palliative Care – We support the individual to be comfortable in their own home and coordinate with other health professionals to provide a service which is seamless.
